Client loss intake

Fill this out when reporting water, fire, mold, storm, or other property damage. Having this information ready helps us understand the loss, prioritize emergency needs, and prepare for the initial inspection.

Active emergency? If there's an active fire, suspected gas leak, electrical hazard, or structural collapse risk, leave the area and call 911 first. Then call us at 888-929-0045.

1Customer & property information

Who and where. So we know who to reach and where to go.

2What happened?

The event itself, when you noticed it, and whether the source is under control.

3Areas & materials affected

Rooms and materials touched. Helps us pack the right truck.

4Insurance information

Optional, but speeds up direct billing and adjuster coordination.

5Emergency actions already taken

Check anything that's already been done.

6Photos & documents to have ready

Have these on hand at the inspection. You can email them to service@aquarestoration.tech.
  • Wide photos of every affected room or area
  • Close-ups of the source and visible damage
  • Photos of standing water, staining, damaged flooring, walls, ceilings, cabinets, and contents
  • Insurance policy / declarations page, if readily available
  • Claim confirmation, adjuster contact information, and carrier correspondence
  • Receipts or invoices for emergency work already performed
  • Plumber, roofer, HVAC, or other contractor report, if available

7Safety questions

Answering "Yes" or "Unsure" won't slow anything down — it tells our crew what PPE and equipment to bring.
